Creating Accessible Surveys and Forms

Accessible forms ensure that everyone, including people using screen readers or keyboard navigation, can provide their input. Regardless of which tool you choose, follow these four core principles:

  1. Avoid Complex Question Types: Steer clear of “drag-and-drop,” “sliders,” or “hot spot” questions. They may be fun but these are often impossible to navigate via keyboard or screen reader. Stick to multiple-choice, checkboxes, and text entry.
  2. Use Clear and Concise Labels: Ensure every form field has a descriptive label. Instead of just saying “Date,” use “Event Date (MM/DD/YYYY)” to provide clarity for all users.
  3. Provide Alternative Text (Alt-Text): If your survey includes images or charts, you must provide a text description. This allows users with visual impairments to understand the context of the image.
  4. Check Color Contrast: Ensure your text is easy to read against the background. Avoid using color alone (e.g., “Required fields are in red”) to convey meaning, as this is not accessible to color-blind users. Check out WebAIM for help checking your contrast.
